The PACE-Gerontology Program invites you to a webinar featuring SF State Gerontology Alumna Ariana Alex, MA, CMC.

Working with a multi-disciplinary team of professionals, under the supervision of the Clinical Care Management Lead, and in partnership with an assigned care manager, supports the provision of comprehensive care management services, while continuing to develop the knowledge base and skills required to take on progressively more responsibility within the Care Management service area.
